Singapore’s National Environment Agency has given notice to all arriving vessels that have been to the Ebola affected Guinea, Liberia and Sierra Leone in the past 21 days that they must submit the Maritime Declaration of Health Form to the Port Health Office.
The Compulsory Maritime Health Declaration by vessels that have been to Ebola affected countries comes after the UN's World Health Organisation (WHO) confirmed a fatal case of EVD (Ebola virus disease) in Liberia, seven weeks after the country was declared Ebola-free.
The ship's form is to be submitted to the office of Capt Danish Gamiest, Port Master, Maritime and Port Authority of Singapore.
The one page form, to be filled by the master or the ship's surgeon, first deals with details of the ship, its sanitation certificates, and requires as list of the times of ports of call within the previous 30 days.
Upon request of the competent authority at the port of arrival, list crew members, passengers or other persons who have the joined ship/vessel since international voyage began or within past 30 days, which ever is shorter.
Questions on the form include: "Has any person died on board during the voyage otherwise than as a result of accident?
"Is there on board or has there been during the international voyage any case of disease which you suspect to be of an infectious nature?" the form asks before requiring particulars.
"Has the total number of ill passengers during the voyage been greater than normal/expected? Is there any ill person on board? Are you aware of any condition on board which may lead to infection or spread of disease?" the questionnaire continues.
"Has a sanitary measure (egg, quarantine, isolation, disinfections or decontamination) been applied on board? Have any stowaways been found on board? Is there a sick animal or pet on board?"
